On May 9, 2016, at approximately 2:30 PM, an employee was carrying a loaded, 25- to 30-pound tool bag while descending permanent, inclined stairs. When he reached the sixth stair tread from the bottom, he lost his balance and fell, striking his right foot against the bulkhead beneath the stairs. He fractured his right ankle and required surgery.
