On the morning of June 24, 2016, at approximately 1:00 a.m., an employee attempted to lift a fan assembly off of a shipping pallet when the chain from the hoist became entangled or loose, causing the housing to tip off center. The employee tried to keep the housing from falling, and his left-hand fingers became caught between the housing and shop floor. His left ring fingertip was amputated.
