On June 30, 2016, a work crew was preparing to offload an electric utility pole from a trailer to install it into the ground. As they attempted to roll the pole off the trailer, a hook-shaped copper ground on the pole caught and wrapped up the safety vest of the injured employee, causing him to be pulled toward the rolling pole. He tried to free himself, but the pole landed on his lower right leg, breaking his fibula and dislocating his ankle.
