While delivering product to a customer location, the employee's foot slipped off of a second level of concrete on which the ramp was set up. He attempted to regain his balance by using the hand truck (dolly) to stabilize himself, but the hand truck fell and his thumb was pinched between the ground and the hand truck. This caused a portion of his left thumb to be amputated just above the knuckle (just behind fingernail).
