An employee was working on the extrusion line. During the process of removing the shaft from the roll, the employee used his right index finger to release the air from the shaft. He pushed the button to move the shaft carriage system upward with the left hand. The shaft carriage system crushed his right finger between the shaft carriage system and the shaft, amputating his index fingertip.
