On September 21, 2016, at approximately 7:30 a.m., an employee and two coworkers were conducting routine checks on the plating line when a piece of debris was noted riding on top of the running strip line/plating solution. The employee attempted to remove the debris from the running strip line with a 12-inch spud wrench when his right hand was pulled into the rotating plating rolls, amputating his index finger, breaking his ring finger, and causing an abrasion to his hand.
