An employee was using a forklift to empty a self-dumping trash hopper into a roll-off box. The employee exited the forklift and approached the back-right side of the trash hopper to release the lever and initiate the dumping movement. The lever released, but the hopper did not empty. He then approached the opposite side of the forklift and placed his hands on the hopper to initiate the movement. The hopper rocked and caught his gloved right little finger, resulting in a crush injury and fingertip amputation.
