An employee was hauling sand in a dump truck. He dumped the truck load of sand and was attempting to close the tailgate on the back of the truck. The tailgate failed to close completely. The employee attempted to pull the tailgate away from the vehicle to create momentum from the swing to close it. His hand was positioned on the bottom of the tailgate and as it swung toward the truck, it created a pinch point. His left hand was pinched between the tailgate and the vehicle frame, causing fractures, cuts, and bruising to the fingers.
An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.
