An employee was extending a scaffold being used by employees to lay bricks for a school wall. As the employee was bolting down the scaffold towers, his wrench slipped off the bolt. This caused the employee to fall over the front of the scaffold and over the brick wall being built. The employee suffered a possible hip fracture and a dislocated shoulder.
