On October 28, 2016, at 6:30 a.m., an employee was removing a piece of stuck steel from the shear. The machine was activated and a 1 inch diameter piece of angle steel was propelled into the employee's left forearm. The employee was hospitalized and had surgery. The machine was not guarded at the time of the incident.
