On January 31, 2017, an employee was removing teeth from a core barrel that was used to drill a 30-inch diameter eight-foot deep hole. The employee was facing towards the hole when he was witnessed as experiencing seizure type symptoms. The employee turned around and looked upwards. The employee then backed up and fell into the hole. The employee suffered a broken neck.
