An employee was threading a paper roll that was not moving. She went to the front of the roll to grab the tail. As she did so, her foot bumped the foot pedal and actuated the paper roll. The rolling mechanism then started up and her hand was caught in the middle and struck by the paper roll. Her left index, middle, and ring fingers were smashed, requiring surgery.
