On the morning of February 20, 2017, at approximately 7:37 AM, an employee was using an emery cloth to polish a small copper electrode part on a lathe. His glove or the cloth became caught in the lathe's rotating chuck, which then avulsed his left ear and face, caused an abrasion on his left hand, and broke his left clavicle.
