An employee was troubleshooting a press machine. When maintenance was complete, the employee restarted the equipment. During the first cycle, the equipment had bolts inside the break. As he was troubleshooting the problem with the bolts, his coworker pushed the cycle button, which caused a rod to push through the equipment. The employee's right hand was resting between the rod and stationary machine. When the rod came through, it amputated his right index finger to the first knuckle.
