An employee was using a forklift to empty an aluminum scrap hopper. He exited the forklift and walked under the elevated hopper. His left hand was on the fork as he struck the lever that dumps the hopper bin.
When the hopper dumped its load, it created a pinch point between the hopper and the forklift forks. The employee's left fourth digit was caught between the hopper and the forklift and had to be surgically amputated.
