On September 15, 2017, at approximately 7:00 a.m., an employee was operating a paint machine when the paint wand hose sprang a leak and began discharging paint at high pressure. He was attempting to grab the wand gun handle when his right index finger was struck by the high-pressure paint (2,000 PSI). His right index finger had to be surgically amputated.
