On November 30, 2017, an employee was performing a daily inspection of the spiral conveyor at the employer's retail distribution facility. To access the drive chain, the employee crawled beneath the conveyor and used his left hand to brace himself as he performed the inspection. He had a rag wrapped around his left hand that was positioned near the moving chain. The rag became entangled pulling his hand into the moving chain and amputating his left middle fingertip. The conveyor was not locked out or guarded at the time of the incident.
