An employee was cleaning around the base of "puck" machinery. He was standing on the railing of the equipment when he lost his balance and started to fall. As he attempted to brace himself for the fall, his right hand inadvertently went into the machine opening that ejects parts, resulting in an amputation of the middle and ring fingers.
