An employee was working on a concrete wall splitting production line. When he turned to discard waste into a hopper, he lost his footing on the raised platform and fell forward into the waste material in the bin. His right middle finger was pinched between the concrete waste in his hand and the concrete waste in the bin, and the tip was amputated.
