Final narrative
A bolt sheared in the middle of a 3-ton feed mixer. An employee was attempting to align the hole with a punch while another employee hit the switch on and off. This on/off action pulled the first employee down into the feed mixer. The employee was hospitalized with a broken right shoulder and arm, fractures on the right side of the face, and head lacerations on the right side that required stitches.
