A maintenance employee was troubleshooting a strapper machine and asked the machine operator to place the machine in "Service Mode On." However, the operator placed the machine in "Service Mode Off." The maintenance employee entered the machine and triggered the service eye, causing the machine to cycle. The machine then struck his foot, piercing his safety boot and resulting in the amputation of his right pinky toe.
