On January 3, 2019, at 10:45 a.m., two employees located at ground level pulled a duct transition piece out of a pit conveyor through the pit conveyor ladder access area. As the first employee was exiting the pit, he put his left hand over a 2-inch metal drip edge that goes around the pit access area. At the same time, the second employee was setting the transition down next to the metal drip edge, striking the first employee's finger. The first employee's left middle finger was pinched between the duct transition and the metal drip edge, suffering a partial traumatic metacarpophalangeal amputation.
