Final narrative
An employee was removing an idler roll from a lathe using a hoist. The idler roll was rigged using a single nylon web strap that was attached to the idler roll with a choker hitch. When the idler roll was lifted, one end of the idler roll dropped. The employee reached out to catch the roll and the tip of his left little finger was crushed between the idler roll (200 lbs.) and the lathe. The finger was partially amputated.
