On February 15, 2019, an employee was using a rolling ladder to retrieve a chair from the top shelf in a warehouse area. He fell while descending the ladder, first landing on the box containing the chair (where he broke three ribs), then landing on the concrete floor (where he struck his head and suffered a concussion). He was hospitalized.
