An employee was cleaning a rotary die cutter machine, which takes sheets of corrugated cardboard and cuts out pizza boxes. The employee was cleaning out ink that accumulates at the side section of the ink metering rolls and suffered a partial amputation of the right index finger below the nail. The machine was not locked out at the time.
