Final narrative
An employee was standing on an A-frame ladder. While transitioning from the ladder to the top of a transformer unit he slipped and fell off the ladder. The employee landed on their left hip and the back of the head resulting in a left hip fracture requiring surgery, and a laceration to the back of the head requiring staples and stitches.
