An employee was operating an overhead bridge crane to move steel beams. The employee placed a beam on the floor to be staged, then unhooked the clamp from the beam. When the hoist was raised, a hook from the clamp caught on the flange of the beam causing the beam to fall onto the employee. He sustained bilateral open femur fractures and a pelvis fracture.
