An employee was using a wire-end brush attachment in a pneumatic die grinder to clean debris out of a casting mold. The employee stepped down from a 6-inch raised platform, then felt severe pain in his left knee as soon as it was bearing weight. A 1-inch-long, 0.0104-inch-thick steel wire from the brush had penetrated the knee. The employee was hospitalized.
