Final narrative
An employee was adjusting a bottle conveyor screw when his wrench slipped and fell into the clutch box assembly underneath the conveyer belt. He removed the face plate of the clutch box assembly and retrieved the wrench. Upon restarting the line, he received a clutch jam notification that stopped the machine. He went back to the clutch box assembly and attempted to determine what was causing the clutch jam. He asked a partner to intermittently operate the human machine interface that starts and stops the machine as he observed and adjusted the clutch mechanism and sensor. As he was troubleshooting back and forth between the clutch mechanism and the sensor, his finger became caught in the clutch gear and belt, resulting in a medical amputation to the left index finger s first digit.
