Final narrative
Employees were working on a press machine that had a mold that was leaking rubber. The employees proceeded towards the control panel to manually lower the mold. The injured employee entered the machine between the front knock-out slides, with a flashlight and approached the mold. Upon starting the inspection of the donut, the employee heard a suction sound. The employee was startled as the top plate came down and he quickly turned his head down and away from the front side of the machine. The employee struck his forehead on the left side rails and then fell backwards and struck the backside of his head on the opposite rail. The employee received staples to the back of the head and stitches to an eye and was hospitalized for a concussion.
