On February 19, 2020, an employee was adding chemical to an alcohol wash holding tank when alcohol wash splashed onto the employee's clothes. The employee put the shirt on the ground and tried to burn off the alcohol with a torch. The shirt caught fire and set the employee's pants on fire as well. The employee suffered burns to the hands, arms, chest, and legs.
