An employee was knocking risers off of castings using a sledge hammer. As he was swinging the sledge hammer, his left index finger was smashed between the sledge hammer handle and a part of the casting. The injury resulted in a soft tissue defect with an amputation at the distal aspect of the left index finger (no bone loss).
