An employee was unloading a pallet jack from the lift gate of a box truck. The pallet jack was caught on the lip between the lift gate and the box of the truck. When the pallet jack moved over the lip, the employee fell from the lift gate to the concrete parking lot below. The employee was hospitalized with fractures to vertebrae and ribs.
