Two employees were hand-loading fiberglass panels onto a hand cart when four panels slipped off the cart to the ground. They tried to pick up all four panels at once, but realized that the stack was too heavy and started to put them back down. At that time, one of the employees lost his grip and the panels landed on his left middle finger, resulting in the medical amputation of his fingertip.
