On September 28, 2020, while walking in the facility, an employee stepped on a tacky mat and one shoe stayed stuck, causing her to lose her balance and fall forward. She landed on her left hip fracturing it and lacerated her left index finger as she tried to catch herself by grabbing onto the lockers. The employee was hospitalized for surgery.
