Final narrative
On October 1, 2020, around 6:30 PM, the injured employee was operating an order picker to fill orders. Another employee struck a pallet, which was then pushed into the injured employee's order picker, causing the injured employee to lose balance and fall approximately 15 feet to the ground. The injured employee sustained a brain bleed and fractures to the skull, ribs, arm, and shoulder. Fall protection was not worn at the time.
