At about 10:30 a.m. on November 27, 2020, an employee was performing maintenance on an exhaust fan in a mechanical room. As the employee tried to grease bearings in the fan, residual energy caused its pulley to rotate. The employee's left middle finger was caught between the pulley and the belt, and the fingertip was partially amputated.
