Final narrative
A fork truck had been used to place metal concrete forms in place. An employee went to manually adjust the "L" sections of the forms by lifting one end with his right hand. The employee was setting down the end of the metal form when it slipped out of his hand pinching the middle fingertip on their left hand. The employee's fingertip was amputated without bone loss.
