The injured employee was in a 7- to 8-foot deep trench installing a large, 66" diameter reinforced concrete pipe. An excavator bucket came loose and fell into the trench, striking the employee on the left side of their body. The employee sustained a large contusion on the upper left leg, a broken left arm, and their left hand was surgically amputated.
