An employee was trying to put the strap on the quick-connect on a bucket excavator so it could pull a pipe out of the ground. As he was trying to connect with a pin, the pin slipped out and his finger slipped into the pinhole. He suffered amputations to the right ring finger down to the knuckle, as well as the tip of the middle finger.
