Arizona Public Service Four Corners Power Plant

An employee was performing maintenance on a bag house compartment. During installment of the lock-out tag-out, the inlet poppet valve would not close remotely. The employee attempted to manually close the valve when it closed on his fingers, resulting in a partial amputation to his left index finger.
