On November 5, 2021, at approximately 8:30 PM, an employee was operating a reach truck to dispose a scrap walkie-rider into an open-top dumpster. The reach truck was operated on an uneven and damaged surface, with an elevated load, causing it to overturn. The employee's head impacted the concrete surface, resulting in a head contusion and five broken ribs.
