On November 11, 2021, an employee was performing die setup on a hydraulic press. The employee loaded dies into the press and actuated it to verify proper die alignment. The upper and lower dies made contact resulting in die failure. Fragments of the die struck the employee's hand causing a severe laceration and fracture to their index finger. In addition, fragments of metal were embedded into both hands of the employee's hands. There was no machine guarding in place and energy control was not performed during die set up.
