An employee had positioned a 12-foot, 144-pound stainless steel round bar on a cantilever arm from the material storage rack. While the employee was pulling the bar into position, it dropped 12 inches off the arm to the ground, crushing two fingers on the employee's right hand. One of the employee's fingers had closed fractures, and the second finger had open fractures, resulting in an amputation at the first knuckle.
