An employee was operating a brake press with a sheet of metal in the press. After bending down to tie his shoe, he tripped over the foot pedal box and fell into the machine. His hands landed under the press clamp and his foot contacted the clamping mechanism pedal. This caused the press clamp to crush three of his fingers, resulting in two fingertip amputations.
