Final narrative
An employee was flowing a gas well. An ice plug formed in the line (upstream) and restricted pressure downstream. The employee began bleeding off the line downstream and the ice plug released. This caused a pressure difference, sending the ice plug downstream. The ball valve that was used to bleed the line was blown off, striking the employee in the right bicep and causing a laceration. The employee was hospitalized.
