An employee was positioning a 3-foot reinforced steel pipe in a pond. The pipe had been lifted with an excavator using a cable hook to hold a chain. The chain was wrapped around the pipe in a choker position. While the employee's right hand was on the pipe, about 6 inches from the chain, the excavator moved forward and the chain caught his hand. His thumb was amputated.
