On March 20, 2022, three employees were installing a small cylinder on a roll stand machine. They were attempting to install the pin that secured the cylinder. The cylinder dropped while one employee's right index finger was in the pinhole of the roll stand machine and partially inside the eye hole on the end of the cylinder. This created a shear point that amputated the employee's fingertip.
