An employee picked up a sheet of aluminum plate with a crane to load the sheet onto a plasma cutting table. The employee climbed onto the table to remove a bow from the plate so it could be laid down flat. The plate was leaning over-center away from the employee. He put his foot on the bottom edge of the plate, at the bow in the middle of the plate, and then directed the crane toward himself. The chain hook holding the plate grab turned past 90 degrees, damaged the crane hook clasp, and pulled the plate grab handle out of the crane hook, causing the plate to fall. The employee saw the plate falling and tried to jump off the table. The plate hit him, pinching his left leg on the table and fracturing it below the knee.
