An employee was helping troubleshoot a relief valve that controlled a safety rail. The employee was descending a ladder on a feed truck when the relief valve was released. The pneumatic rail amputated the employee's right thumb, which caused the employee to fall 4.5 feet to the concrete floor and suffer broken vertebrae and head lacerations.
